Shoe Fitting and Free Pair of Diabetic Shoes and Inserts for Diabetics
If you are a Diabetic and have Medicare or another primary insurance, you are eligible for a free pair of diabetic shoes and inserts. You must have your PCP fill out the doctor's order form which should be picked up at the Jenks asap PRIOR TO YOUR FITTING ON MARCH 31ST.

Call the Jenks at 781-721-7136 to make your 10-minute fitting appointment. Appt. is required! YOU MUST PICK UP PAPERWORK* FROM THE JENKS ASAP PRIOR TO YOUR APPOINTMENT ON 3/31.
*Please note: You must obtain a doctor’s order form from THE JENKS CENTER and you must have your doctor complete that specific form PRIOR TO THE FITTING ON MARCH 31ST and bring with you to your appointment. You will NOT be eligible for this service without the completed doctor’s order form and all other necessary paperwork.
